Our Patient Care Coordinators

As a Patient Centered Medical Home, McCook Clinic employs Patient Care Coordinators to help our patients stay healthy. Patient Care Coordinators help to ensure you are scheduled for important tests and screenings, answer questions about medicines you are taking and relay important information back to your doctor. One of our Patient Care Coordinators may call you after or between visits to remind you about tests and screenings. They are here for you!

Gayla Hasenkamp, LPN - Care Coordinator

Gayla joined the McCook Clinic in 2016. She has worked in many healthcare settings throughout her career and this experience has given her great insight into coordinating care for our patients. Gayla has been invaluable as our Chronic Care Management Specialist.  This outreach of care is proving to be just what we needed in Southwest Nebraska.

Mika Lentz, LPN - Care Coordinator

Mika joined the McCook Clinic in 2015. She has been a Licensed Practical Nurse for 12 years and has worked primarily in the clinical setting. Many years of hands-on nursing experience gives her a better understanding of our patients needs. Mika's role on the Care Coordination team focuses mainly on transitioning our patient's care back to the clinic from recent ER visits and hospital admissions.

Brooke Brooks, RRT - Pre-Planning Specialist

Brooke joined the McCook Clinic in 2013 and worked on the nursing floor until transitioning into the Pre-Planning Specialist position in August 2019. Brooke has been a Registered Respiratory Therapist since 2004. Prior to coming to McCook Clinic, she worked at Community Hospital monitoring overnight sleep studies and also spent 7 years working on the PICU and NICU floors at Children's Hospital in Omaha. Her primary goal as Pre-Planning Specialist is to find gaps in care and do her best to close them. This helps ensure that our patients care is comprehensive and their medical needs are met.
