Our Patient Care Coordinators
As a Patient Centered Medical Home, McCook Clinic employs Patient Care Coordinators to help our patients stay healthy. Patient Care Coordinators help to ensure you are scheduled for important tests and screenings, answer questions about medicines you are taking and relay important information back to your doctor. One of our Patient Care Coordinators may call you after or between visits to remind you about tests and screenings. They are here for you!
Gayla Hasenkamp, LPN - Care Coordinator
Gayla joined the McCook Clinic in 2016. She has worked in many healthcare settings throughout her career and this experience has given her great insight into coordinating care for our patients. Gayla has been invaluable as our Chronic Care Management Specialist. This outreach of care is proving to be just what we needed in Southwest Nebraska.
Lorie Berry, LPN - Care Coordinator
Lorie joined the McCook Clinic in 2003. Before transitioning to the care coordination team, she worked for many years on the nursing floor. These years of hands-on nursing experience gives her a better understanding of our patients needs. Lorie's role on the care coordination team focuses mainly on transitioning our patient's care back to the clinic from recent ER visits and hospital admissions.
Abby Larington - Pre-Planning Specialist
Abby joined the McCook Clinic in 2020. Before joining the clinic team, she worked as a CNA for 5 years. Being born and raised in McCook has made her familiar with the community and drives her desire to be an integral part of the care team here at the clinic. Her primary goal as Pre-Planning Specialist is to find gaps in care and do her best to close them. This helps ensure that our patients care is comprehensive and their medical needs are met.
